- Amyotrophic lateral sclerosis (ALS)
- Crohn’s Disease
- HIV (Positive Status)
- Multiple sclerosis (MS)
- Parkinson’s disease
- Post-Traumatic Stress Disorder (PTSD)
- A terminal condition that is diagnosed by a physician other than the qualified physician issuing the physician certification Chronic Pain (Nonmalignant).
